Description

In this digital and printable 42-question student behavior self-assessment, students will read a list of behaviors based on real classroom experiences and assess if they are inappropriate, appropriate, destructive, or unsafe behaviors. This survey is designed to increase student awareness of the impact of their behavior and promote engaging classroom discussions.

This assessment is perfect for special educators needing documentation for Functional Behavioral Assessments or IEP goal setting. I use it at the beginning of the year to gauge student understanding of their own behavior. Their responses can be used to facilitate classroom discussion or one-on-one conferences.

I remember making that up for a group of kiddos who didn't have the concept of physical boundaries. I hope it helped you with your class. Thank you for your feedback.

Megan, I want to let you know that your feedback was extremely helpful. Thank you. After careful evaluation of the key, I decided to eliminate it. When I conference with the student about the reason for their answers on this assessment,I always review the answers with students, and I find their explanations very helpful and sometimes just plain insightful. If I'm using this as a tool for a functional behavioral report, a behavioral intervention, this conference piece is still vital because it helps me better understand the student's thinking. I really like this tool, but maybe having an answer key is more rigid than it needs to be. Thank you for your invaluable feedback. Mia
