This behavior Reflection form can be used daily, weekly, monthly, however you see fit for your students! It is a quick way for the students to reflect on their behavior that includes, you and the parents so there is communication between everyone. There are simple statements with visuals for the students so this can be completed independently if needed. Once the students have completed it in pencil the teacher goes through and also circles in pen, so the parent can see how both you and the student felt about the behavior for that time period. There is a place to add comments and for the student, teacher, and parent to sign. This can be used as a quick end of the day reflection or something used once a quarter for conferences.
