This is a behavior sheet I created for my students to take home every night. It correlates with my clip chart tally mark behavior sheet. Green- 0 marks, yellow -1 marks, bottom of yellow -2 marks, orange -3 marks, bottom of orange -4 marks, and red -5 marks. I had an editable clip chart I used to go along with this in my class as well. I downloaded it from another TPT user. Hope this works for you it definitely did for me!

To save time at the end of the day, I had students sign this on their own but if they were on a color besides green, they had to check in with me before putting it in their take home folder.

You can also use it as a tool for only specific students in the class as well.
