Use this student check-in / self-reflection survey with your students, either once or multiple times throughout the year (check out our store for more themes)!
This survey asks students how they are feeling overall about school, peer interactions, their stress levels, and if they are feeling overwhelmed. It also gives teachers a check-in point for each of the core subjects, including Language Arts, Math, Science and Social Studies.
Students are given the opportunity to reflect on their school year so far and name projects/assignments/tests they are proud of completing. It also gives students an opportunity to discretely communicate with their teacher if they are having issues in a class, or with something else at school. This will be useful for students who are shy or nervous about speaking with the teacher
The sassy lemon theme keeps the survey light and fun while also offering visual cues for what some of the questions are asking. There is also a bonus question on the second page so students can use their creative sides!
