Student Goals Progress Chart

Description

The Goal Progress Chart form is an easy way to keep track of your students' progress throughout the year! It is part of the Student Goal Folder program and the first page in the student folder. Use the Goal Progress Chart when conferring with each of your students. It's a quick reference and provides an overview of each student's goals, performance levels, and progress in reading and math (basic fact fluency). The form is updated three times during the year (Fall-baseline, Winter-1st Semester, Spring-2nd Semester). It is filled out by the teacher and referred to when meeting with each student to determine goals for the next grading period. The Goal Progress Chart can be shared at conferences, too! It's an excellent document that gives parents a quick overview of their child's growth and progress.

The Goal Progress Chart includes a place to record the following:
* Reading Level (Just Right Reading Range)
* Dibles Score (Words Per Minute)
* DAZE (Comprehension)
* Battle of the Books Read (# of books read that grading period)
* Accelerated Reading Goals (Point Goal & Comprehension Goal)
* Basic Fact Fluency Timed Tests (Addition, Subtraction, Multiplication, Division)
