My digital student intake form has helped me quickly know a bit more about my students, their needs, and their excitement for the upcoming school year. I am excited to share it with you!

This digital form asks your students three important questions, "Who are you?", "What type of learner are you?", and "What should be in our class contract?". Through this resource you will learn about what excites your students, what troubles your students, what they like to do outside of class, and what kind of learning environment they want to participate in.

The added bonus of this resource is easy accessibility is once students complete this Google Form, you can create a easily searchable Google Sheet to review their responses!
