This Google Form™ survey is a great first day activity. Get to know your students with questions about themselves, their academic styles, and their personal interests. There is a name pronunciation question included, allowing you to hear your students pronounce their names themselves, creating a saved audio clip that you can go back to.

The questions are at a level appropriate for Grades 7-12.
