This one-page sheet of paper (front/back) makes it easy to record student growth for one assessment/topic/skill/standard. The form includes sections for:
- describing what Mastery and Near Mastery look like
- recording student scores for up to four assessments to measure growth
- writing notes/reminders/reflections
- recording subgroup scores separately
This sheet provides an excellent reference from year to year as you push for higher levels of mastery.

This is my most basic data sheet. It is designed for Elementary Teachers (or secondary teachers who only have one roster) and it will calculate average score and how many students are at each level of mastery.
This is similar to my most basic data sheet, but it is designed for PLCs of up to six teachers. Each teacher has their own tab to enter data, and the sheet also has a tab that compiles data from all six teacher tabs. In this way teachers can compare data using their individual tabs, and they can see how the entire group of students are doing in that grade level, etc.
This data sheet is designed for Secondary Teachers with multiple class sections. There are enough tabs for eight class sections, and another tab that compiles data from all of the sections.
Do you want to track subgroups?
Subgroups may include traditional subgroups (IEP, FRL, etc.), but they are fully customizable, which means they could be used to track reading groups, intervention groups, etc. Here are the sheets that help teachers track subgroups:
Elementary Teacher w/ Subgroup Tracking
This data sheet is for one teacher (one roster) and allows the teacher to track up to six subgroups.
Elementary PLC w/ Subgroup Tracking
This data sheet is for a PLC (up to six teachers) and allows each teacher to track subgroups for their class, and the data from all subgroups is compiled on a separate tab.
Secondary Teacher w/ Subgroup Tracking
This data sheet has tabs for up to eight course sections, and it calculates subgroup data for each section while also compiling data for all sections combined.
Are there other options?
This option is available if you want a data sheet to make your life easier, but none of my standard options really meet your needs. If you download the PDF you will see a pricing guide, but this is only a guide. For example, if I can quickly modify an existing sheet to fit your needs then the price will be less. If I have to create a sheet from scratch, the price will be higher.
Why isn't there a secondary level PLC data sheet?
I try to make all of my data sheets generic enough that any teacher in any district can use it. For example, all of my secondary data sheets have enough tabs for eight course sections (I don't imagine that any teacher has more than eight sections). To make a generic PLC data sheet at the secondary level would look like six tabs for teachers, and each tab would have to have enough space for eight course sections. In other words, the data sheet would be so huge that it would become difficult to navigate. So if you would like a PLC data sheet at the secondary level, I can create one to your specs and it probably won't be so huge. Use the Customized Google Data Sheet option above.
