This is a referral form my students complete to report minor incidents that don't need immediate attention. It is designed to cut down on tattling. If the incident isn't worth the effort it takes to fill out the form, it isn't worth my time to handle it. The form also gives me a written record to refer to when I do need to deal with discipline issues.

I really liked this resource. It has cut down on the tattling that was occurring in my classroom, and it forced those tattling to be truthful. Now I only get legit complaints because students know they will need to log the compliant to keep record of it.
