My students use this sheet to reflect on their learning targets for the week. I also provide them with a place to put their peach and pit for the week, which is their high point and low point. Then they have a spot to write to the teacher about anything.

These are super useful for me as a teacher and it gets the students into the habit of thinking about themselves and their learning.
