Introduce this Google Form at the end of each marking period/semester to allow your students to reflect upon their participation and involvement in their classroom. Students will self-rate and give themselves a citizenship/attitude score — this means reflecting on how well they are meeting overall class responsibilities, participation, classwork completion, homework completion, etc. Google Forms makes it easy to use this reflection anytime, anywhere, and will ensure no papers are lost or misplaced!
What is included in this product:
1 EDITABLE Google Form:
The categories students will score themselves in are as follows:
- O = Outstanding: ABOVE and BEYOND every day in school
- S = Satisfactory: MEETING Expectations most of the time in school
- N = Needs Improvement: Should do more in school
- U = Unsatisfactory: Not fulfilling any of the responsibilities in school
- They will choose this score after filling in both short answers and scale questions about their marking period/semester
- The form also includes goal setting questions to set after reflecting!
You will automatically be given a copied EDITABLE Google Form, so you can quickly personalize my given template to suit your teaching style. Once complete, I suggest you convert your responses to a Google Sheets format, so that you can keep all of your data in one place. Quick and easy!
*Please note this Form relates less to student’s grades (while it does ask once or twice about academics) and more to their efforts and responsibilities in class throughout the marking period.*
