Description
The Student Responsibility Plan allows students to take ownership of their actions and devise a plan to improve behavior. There are four forms per sheet. I highly encourage printing these on colored paper. I usually have a stack printed and cut in one of my classroom trays. I warn students once of their behavior. If they continue, students know that they will be told to complete a Student Responsibility Plan. In addition, the form includes a section for both students and parents to sign. This keeps parents informed of their children's behavior and usually helps build collaborative relationships with the teacher. I have found that students who complete forms early on in the year, get better as the year progresses in terms of behavior. This is probably due to three factors, students don't enjoy filling out the form and giving it to their parents, parents talk to their children about the behavior, and students actually think about their behavior when filling out the form and/or speak to their parents about it.
