Description

This student quarterly self-evaluation is a great way to encourage reflection, set goals, and foster communication between the student, parent, and teacher. This is great to have for conferences or to start a new semester!

This PDF now has an interactive layer so you can assign it to your students online.

*Students will access this version via Google Classroom OR a Google email account.

This resource includes 3 different evaluations for upper elementary through high school.

The first document is simple and to the point. Students identify their strengths, areas they could improve, and rate their overall effort in class. I used this document when I taught middle school.

The second document is more detailed, with a specific area for goals and a plan to reach these goals. I used this with my 8th and 9th graders.

The third document is even more detailed, with a rubric the students use to rate their effort, organizational skills, performance, and behavior. There is also a section for goal setting where students set goals, determine actions to meet them, and ask for specific help needed from the teacher. I used this with my high schoolers.

On the third document, there is a section for parent and student signatures that could be used during/after conferences or just as a document to keep in student files.
