It can be difficult at times to keep track of students who leave the classroom. This sheet makes it easy. A "destination" column is included on this sheet. If a student asks to go to the washroom and gets in trouble off school property, this will make it hard for him/her to claim that you consented to him/her leaving the premises.

My students often leave the room for other services, pick up materials from gen ed teachers, or even just to use the restroom. This sheet was very simple, but effective in tracking where my students were going and how long they were gone.
