Description

Want a quick way to discover information about your students in your math class? Give them a short survey using Google Forms. This form is geared for upper elementary school students but could easily be modified for Middle and High School students.

Questions included on the form:

Section 1: Sample Information about you

  • First name and nickname
  • Last name
  • What is one thing I should know about you?
  • When you have a few moments to do whatever you want, what do you like to do?
  • Do you think you are smart?  Why or why not?

Section 2: Sample Information about Math Knowledge

  • What level of dedication do you give to learning new math skills?
  • What is your comfort level with math?

Section 3: Sample Information about Classroom Environment

  • How well do you work with your classmates?
  • How much do you like to help others?
  • How much do you like to work in cooperative learning groups?
  • How well do you follow oral directions?
