Description

This form can be used for students to reflect and self-evaluate their learning and work on their study habits for the next assessment. At the end of the form, there is a spot for parents/guardians to write a note to the teacher if they have any concerns that the teacher should be made aware of.

The teacher, student, and guardian will all sign the form making this a "contract". This is to show everyone is in agreement for the retest and keeps the student accountable for their own learning. It is also a way to foster a relationship between the teacher and the parents.

How do I use these forms in my own class?

Depending on the class I allow everyone who got below 70% to retake the test. When students submit their filled in forms I staple it to their original test, and include their retest when they have complete it. I keep all copies for parent teacher conferences and to have on record when reporting on the progress of the students.

Depending how you would like to use it and what your focus is, this document can be edited.
