This PDF includes:
A single-page worksheet for ND kids to fill in with their adult, and then pass along to any other adult who is supporting them (e.g. coach, teacher, therapist, camp counsellor, nurse, etc).
There are 2 different versions: one specifies that the child is Autistic and/or ADHD. The other is generic and doesn't specify any neurotype.
They are both available in colour and B&W (for kids who want to colour them in).
This resource also includes an 8-page document with examples and suggestions of what to write on your 'stuff about me' worksheet.
