This document is a 1-page reader's response I use during reading workshop. It holds students accountable for summarizing their reading and also includes a graphic organizer to help them brainstorm before constructing their response. It can be used for a short picture book, or a longer chapter book. The Beginning, Middle, End boxes on the top lend themselves to brainstorming with students still developing a sense of sequence in their reading.

It has suggested language for transition words as well.
