Technology Use Reflection Form for Student/Teacher Conferences

This Technology Use Check In and Reflection Form is designed to be used during student/teacher conferences and signed by a guardian. The form focuses on appropriate technology use in class, with a particular emphasis on cell phones and how inappropriate use can impact a student's grade.

The form begins with sections for student name, class period, current grade, and date.
