This is a way to get students to learn from their mistakes. For math tests, I always wanted to give students the opportunity to re-learn and fix the mistakes they made on their test. The point is to learn the material, right? So I give them these test correction forms to give them a chance to prove what they have learned.

This paper is just a one page (or as many as you need) form with a place for the problem number that they missed, the new correct answer, and then a written reason why they missed it or a place to show their work for fixing it.

The page is editable so you can adjust as needed. This particular paper was designed for an online test but you can adjust wording if your test is different. I used this same template all year for our tests.
