This form can be used when reviewing an assessment. It breaks down the misconceptions behind why students missed a question and requires them to slow down and consider why the correct answer is, in fact, correct. It's formatted to fit 2-to-a-page on a letter sized page so it can be cut in half to fit in a composition book / interactive notebook. You can also double side print it and have students attach it to their tests.

*Helpful hint: I grade this as an all-or-nothing classwork grade (0% or 100%) - however, they have unlimited tries to explain the items correctly. They must explain the reason behind the correct answer. If they struggle with that, I allow them to explain the process of elimination for the other answer choices. I teach Physics, so a lot of the problems are math based - I require them to work out the whole problem for credit.
