Test Reflection/Correction Form

Description

Do you want your assessments to be more meaningful for your students than a grade in the grade book? This form provides structured reflection and feedback to you and your students. It is in .doc format and is easily customizable to fit your needs. Simply type which assessment you are correcting, the essential standards and question numbers for that assessment, and distribute to your students following a test. Students must first complete a self-evaluation for each essential standard based on the test questions that you list. Then they answer a few brief questions that reflect on your teaching and their learning. This self-reflection is a critical part of their development into independent and mature learners.
On the back of the form, students are prompted to select 5 questions that they answered incorrectly (preferably from the essential standards/questions listed on the front) to both correct and provide a complete explanation of the solution. Given the opportunity to reflect and make corrections to past mistakes is considered one of the most effective means of student learning. This form will provide you with a classroom proven structure to accomplish this with your students.

Although you have this listed for higher grades, I think my 3rds will really benefit from analyzing their reading unit test with this. I will use your 1st sheet twice though as they had 6 standards on the test. Thanks!!
