Testing Observation Form


This form was created for School Psychologists, but can be used by other specialists completing school-based evaluations (speech-language, occupational therapy, special education, etc.)

It is meant to be used during 1:1 testing sessions with the student to record observations of the student and their approach to testing, and then can be later used when writing up observations for the evaluation report.

The front page includes observations related to the students Appearance & Affect (including their grooming/hygiene, clothing, vision/hearing, mood/attitude, and attention/activity level) and Speech & Conversation (including intelligibility, eye contact, conversation, and self-advocacy).

The back page includes a large table with space to record behaviors (positive and negative) on individual subtests - which is super helpful when writing up the evaluation report and interpreting results!
