This is a "Think Sheet." I use this when a student has repeatedly acted out or made the wrong choice(s). I reserve about 5 minutes of time to pull them to my desk so that we can speak one on one to ensure that they know why they have received a Think Sheet. I also have them answer and write in complete sentences. This also helps you as the teacher to document anytime you have reached out to the parent/guardian concerning their child's behavior. I include my own Bitmoji at the top, but you can replace this with your own or erase entirely.
