Purpose
This assessment sheet is designed to track and measure students' progress across core life skill areas as part of a transition program for the school year. It helps teachers evaluate performance at the beginning and end of the year in three major domains.
Main Skill Areas Assessed
- Vocational Skills
- Includes goals like job searching, identifying personal job-related strengths, preparing a resume, and recognizing job opportunities in the community.
- Social Skills
- Focuses on appropriate conversational skills, interpreting social cues, asking for permission, and initiating or engaging in peer activities.
- Independent Living Skills
- Covers money management (e.g., budgeting, tipping, responsible spending) and personal management (e.g., self-awareness, hygiene, organizational skills).
Assessment Format
- Each skill area includes:
- A task objective (the specific student goal)
- A descriptor: “Yes – the student performs independently and consistently” to define expectations.
- Columns for Beginning of Year and End of Year scores (dates and progress notes to be entered by staff).
