I have sent numerous requests for access to this google form that I purchased and haven't received any response. Please grant me access to this so I can let my students do it.

I'm sorry Patty, the only way for me to respond to a buyer is through the TPT question forum. I haven't seen any request for access to this Google Form. But, I've checked the link to the product on multiple devices and they are all able to open it correctly. When you download the PDF, there is a link to the Google Form at the bottom. It will prompt you to make a copy that you can then assign to your students.
