Quickly check for understanding at the end of any math lesson with these simple, effective Universal Math Exit Slips. Designed in a half-sheet format to save paper, each slip gives you insight into student confidence, problem-solving, and reflection — all in one page.

Exit Slip Layout:

  • Name + Date
  • Topic / Lesson Focus line
  • Section 1: Confidence Check → “How well do you understand today’s lesson?” (1–5 star scale: 1 = not at all, 5 = I could teach it)
  • Section 2: Quick Check Problem → space for teacher to add any problem + student work
  • Section 3: Reflection Prompt → students choose one:
    • “Today I learned…”
    • “One question I still have…”
    • “A mistake I corrected was…”
