I recommend you use this to confirm what time a student left class for your records.

4. Google forms Virtual Restroom Sign Out Sheet with QR Code directions
In order to create a QR code you may conduct a google search and several will populate. I am not at liberty to post any QR code links to websites because I am not affiliated with their business entity.
