This sheet can be utilized for any student who has lower reading and writing abilities, and still allow for them to fill out a daily assignment notebook with homework and dates for tests and quizzes. I use this with one of my Resource students who is Dyslexic and has difficulty reading/ writing. This is a modification to our students' agenda book which is expected to be filled out daily with the homework. The student just circles what type of activity they have for homework (read, write, worksheet, or none) for each class. There is some space left for them to copy down page numbers or chapters.

This has helped my students tremendously with participating in filling out their assignment notebooks, and I hope it can help your students too!
