Description
I use this chart with students whom have trouble following directions. This weakness effects behavior and completion of work in class and at home. I hold on to this (or it is given to the next teacher if the student switches classes) and simply rate the behavior for each class period. Comments can be made from parents and/or teacher at the bottom. Parent initials each night. I check for this, but as long as it is returned on Monday with initials across the week, the student gets to pick from my treasure chest prizes.
