I used this to keep parents informed of any issues that popped up during the week. The codes related to the rules in my classroom. I signed off of the behavior cards daily and then sent them home to be reviewed and signed by their parents on Friday.

I ran these on the large index cards so that they were more durable. I taped the students card on their desk for easy access.
