This editable weekly behavior chart allows students and teachers to set two goals to focus on for the week and decide upon a reward for reaching their goal 7 or more times. I have always called it Check In/Check Out, as the student checks in with the teacher periodically throughout the day to discuss the choices they have been making. At the end of the day, students receive a sticker or smiley face in the box next to any goal they achieved that day. There is also room for teachers to write a note and state whether or not the student reached his or her daily goal (reaching 1 or 2 goals). My favorite feature is the spot at the bottom for students to reflect on their week. There is also a place for a parent signature. It's flexible because you can choose the behavior goals, rewards, and the number of reached goals needed to receive a reward.
