This is a very simple weekly behavior chart used by classroom teachers. It works well with the PBIS program, which our district has all elementary schools implement.
The students use the behavior chart to work towards a daily or weekly behavior goal and reward. The reward can be as simple as reading a book with a younger child at the end of the day or having 5 minutes on an educational computer game.
This is a word document. It can easily be edited to fit your students, their schedules, and specific goals.
