Description
When students struggle behaviorally, teachers are often asked to monitor and track behavior. Often, students can, and should, be active in this task. When they are in charge of the information and have a visual reminder, they own their behavior. This product contains five different weekly tracking sheets to help accomplish this task. They are very easy to understand and do not take much time to complete. Just choose the one with the behaviors the student needs to work on, make a few copies, and set up a system with the student. I have folders in a holder in each homeroom. The students know to go to their folder, get their sheet, and get it filled out by teachers throughout the day. Parents sign or initial each night, and, based on student preference, the students earn rewards from a food item to computer time to basketball/active time. These sheets offer a quick, easy start to getting behavior under control.
This form is also good for tracking changes in behavior as interventions are put into place. The information may be compiled into a spreadsheet based on individual needs.
