Sometimes our students need a little extra help and support. These weekly behavior reports can be used to assist you with providing students and parents with quick feedback.
Simply print the chart. Then the number of stars or the word that describes the student's day (Excellent, Great, Good, O.K., or Challenging). You can have students circle the chart themselves or you can circle for them. There is also an area for them to designate areas for increased focus. There is a place for the student, parent, and teachers to sign at the end of the week.
I am going to be working on different parent communication tools, so if you have any specific requests or would like to have me make a different version for you, please let me know!

PS An interesting note about the star rating: I decided to list give the challenging behavior one star (as opposed to no stars). This way, parents will know that the teacher and/or student did not forget to mark the sheet (which might happen if no stars were circled). By circling one star, parents are assured to receive feedback. I do have one variation included without stars, so you may use the version you prefer. There are also versions with bees, apples, owls, and dolphins to fit your classroom theme.

Helps with my parent communicate and behavior report for my first grade student.
I have used these for 4 years now and they are great!! Easy for students and parents to see how their student did!!
My students loved the print it is good for the students to do themselves and have ownership. Very simple and quick to do with them at the end of the day.
This is JPEG pictures so you can not edit it, would be better if you could edit it to fit your students needs
