Description

This folder includes 8 variations of some weekly behavior tracking sheets. Each sheet includes a place to put the behavior(s) you are tracking and space on the side for several times during the day so you can input the daily schedule of the student. There are two copies of the tracker on each page in order to save paper. I highly recommend including students in the process of developing behavior plans, to whatever extent appropriate for them. Allowing them say in their rewards, understanding specifically what you want from them, and ensuring they fully understand the plan, will increase the effectiveness of any plan. All documents are fully editable to so you can adjust to suit the needs of your students. Specifics are listed below by file name.

  • 2 goals leveled rewards: This sheet has space for 2 behaviors per day to track (listed with abbreviations in the chart- full description is on the side with levels for scoring each). All information for following the plan is listed on the right-hand side along with the behaviors/levels for scoring. Below the requirements you can also see how the student can earn leveled rewards. You can set goals for this appropriate for the student and the reward. These rewards can be set daily or weekly (currently it is set up for weekly). There is also space for consequences of not earning enough points.
  • 3 goals no reward: This sheet would be primarily used to track behavior. There is a place to list behaviors that you want to track at the top, and space to indicate a schedule/activities along the side. Choose 1, 2, or 3 behaviors that are most prominent or concerning. For ease of tracking you can indicate that a behavior was seen by writing the number in the box to show when it occurred. (There are versions below that include rewards.)
  • 3 goals with redirection: This sheet is similar to the tracking sheet above; however, it has places for 3 reward times. There is a place to list behaviors that you want to track at the top, and space to indicate a schedule/activities along the side. Choose 1, 2, or 3 behaviors that are most prominent or concerning. For ease of tracking you can indicate that a behavior was seen by writing the number in the box to show when it occurred. This sheet also has an R1 and R2 in each box. This is so that the first and second redirection within each time of day can be marked before marking if a student has exhibited a behavior. (This is particularly helpful to demonstrate when a student is able to demonstrate appropriate behaviors with 1-2 redirections.)
  • 3 goals: This sheet is similar to the tracking sheet and 3 goal sheet above; however, it has places for 2 reward times. There is a place to list behaviors that you want to track at the top, and space to indicate a schedule/activities along the side. Choose 1, 2, or 3 behaviors that are most prominent or concerning. For ease of tracking you can indicate that a behavior was seen by writing the number in the box to show when it occurred.
  • Front to back leveled rewards: This sheet has space for 2 daily behaviors and a daily reward. Behaviors on this sheet are rated, and the levels earned equal points toward the end of day reward. There is a space for a rating scale on the second page (to rate behaviors) as well as a place to identify leveled rewards based on points earned. This document is intended to be printed front to back so all days of the week are on one sheet.
  • Front to back reward based on minutes: This sheet has space for 3 daily behaviors and a daily reward. Each positive behavior a student exhibits on this sheet earns them a minute toward their end of day reward. This document is intended to be printed front to back so all days of the week are on one sheet. (Given the formatting of this sheet, the easiest way to change behaviors to meet your needs would to be to use the find and replace option within word.)
  • Leaving Room: This sheet is similar to the tracking sheet and 3 goal sheets above; however, it has a place for an end of day reward. This sheet also includes a line at the bottom to track when a student leaves the classroom during the day. This can be used if they leave without permission, or if they ask for a break to leave the classroom (based on your needs). There is a place to list behaviors that you want to track at the top, and space to indicate a schedule/activities along the side. Choose 1, 2, or 3 behaviors that are most prominent or concerning. For ease of tracking you can indicate that a behavior was seen by writing the number in the box to show when it occurred.
  • Self-teacher monitoring: This sheet allows for one behavior to be monitored by a student (self-monitoring) and their teacher. Choose the behavior you want to work on and set the levels at the top. You can also change the symbols to mark the levels.
