Looking for a sheet to send home that has homework assignments AND behavior? Looking for a simple way to keep in touch with parents? This Weekly Homework and Conduct/Behavior Sheet is a super easy way for students to and parents to track homework and behavior.

I have used this sheet for several years with my elementary classes, and it worked very well. It was an easy way for students to keep track of their homework assignments and for parents to do a quick check on their child's academics and behavior. I checked them off each morning for the parent's signature.

I also included is an editable version where you can tailor it to your needs. You can change the subjects, conduct, etc. You could also include notes to parents like Field Trip, Picture Day, or please send Kleenex.

The questions/concerns area is for you or the parents to write down questions or concerns. It's a quick and effective way to communicate with your parents.
