My school does not require that SLPs turn in a full lesson plan every week. Which is wonderful, but I still like to jot down some informal notes of things I want to try with particular students, new problems I want to address, activities that I think would be beneficial, etc. I tried some lesson plan formats, but felt that they didn't really suit my needs as an SLP. This lesson plan template starts with a week-at-a-glance to-do list and then moves into each day of the week. As SLPs, our schedule is never the same from day to day so there is a time slot column for EACH day of the week. It has been an incredibly helpful tool for me and I hope it can help keep you organized as well! :)
This version is in Microsoft Word, which means you can edit it! The font is different than the PDF version, simply because it is not a word supported font. You can type times, names, activities into the slots. You can also delete time slots, or add them, as necessary!
