Use these weekly progress report forms to monitor and track academic progress as well as changes in attitude and behavior. The forms can be used in schools using traditional letter grades and in schools using a standards-based assessment system.
You can use them at conferences with caregivers to show the progress their kids are making in academic and behavioral areas. Just save the forms with other student work to create a portfolio that both the adults and students will appreciate.
Sending these progress update forms home every week also lets caregivers and students see how they are doing in class between report cards and lets them know if they need to make up any missed assignments.
How to use the weekly progress forms:
* For each of the 5 items, circle the appropriate description; you can also write a comment underneath if you wish.
* Add a general comment in the space provided — writing something positive helps foster a positive relationship with your students’ families.
* Send them home on Friday for parents/guardians to look at and sign; there’s space for parents to write a comment, too, if they wish.
* Students bring them back to class on Monday and give them to you or or parents/caregivers email them to you by Monday.
* Save the forms in a binder or folder to show progress over the school year.
These weekly progress update forms are simple to fill out, with short sections addressing:
* Overall Performance
* Quality of Classwork
* Homework
* Behavior
* Attitude

I encourage everyone to read the product description because it lists what's included and to look at the Preview because that shows what's included. The form has 4 versions: printable in English, printable in Spanish, digital in English, and digital in Spanish, so teachers can use it with their Spanish-speaking students' families and the digital version offers convenience if they want to save time and ink instead of printing them out.
