This tool gives 4 clear parts of the math problem solving process for student mastery. It can be used as a guide for students to have a criteria for success, a tool for diagnosing trends of a class/student, and a tool for data collection for IEPs. The thumbnail only shows one row. I have left it as a Google Document so that you can edit for your own needs. It does align with graphic organizers for a variety of topics.
