This folder serves as the parent/student paperwork for the Work-Based Learning program--from liability to photography to insurance. Students are able to fill it out with their parents and return it the WBL Coordinator.

The Coordinator may use the folder to record hours and pay and use it to keep up with any documents the students turn in throughout the year.
