This form is used to help document when students are in class, yet refusing to participate or complete the given assignment.
I use this as a behavior management strategy.... which results in very few forms ever being filled out. When I see a student off task and they refuse to get on task, I bring them the Work Refusal Form. I explain that they have the option to sign the form, or get on task.
No disciplinary measures are taken.
I explain that at the end of the grading period when they fail, this is the documentation I will show their parents.
Usually, this is enough to get students on task. On the rare occasion it isn't, you have documentation for parents and admin of your attempt to redirect the student.
