This teacher note-catcher follows the same process I go through when I confer with my students.
This form tracks three consecutive conferences (Just print a new sheet for the next 3 conferences, etc.). First, I observe and quickly assess certain strategies the writer is already attempting/using during my initial "research/observation phase." Sometimes I notice multiple strategies and this form has space for 5 big strategies.
Then, I move into the compliment, conference, and teach portion which I like to take notes on at the bottom section of this form. Also there is room for a "next steps" jot post-conference.
This resource works well with my other resource "Independent Writing Assessment" (which I use to serve as a cover page for every child in my class).
