This 4 year planning sheet (version 3) should be used for either student, parent, Counselor, IEP Chair, SST Chair, or Administrative use to keep track of course credits and plan for High School. This is a Maryland-specific form, but can be easily modified to accomodate any other state or private school High School Requirements. This Word document is completely editable. This Mental Health Student Survey can be used in the beginning of the year to capture what your students feel about their school climate. It includes 8 questions that can be answered with "agree", "disagree", or "unsure". There are also open ended questions, such as "I think students need help with..." and "comments/concerns" for students to write in their own ideas.
