I created this permission form to use food in my therapy sessions. In my school, outside food is not permitted, so the use of food and a separate form ad to be created and approved by administrators. Feel free to edit as needed!

This contact log is especially good for therapists (or teachers) who have to document any contact with parents. I have included spots to check off whether the contact is via email, phone call, voice mail, or face-to-face meetings. This form is intended as a running record and not for each individual students, but feel free to adapt for your needs!
