This 3 page pack includes color & b/w summary pages that a substitute teacher can use to communicate with a host teacher. The pages include a lesson report (with any modifications,) a student report (great helpers in addition to struggling students,) and a general notes area. This pack is a great help when completing 1/2 days in multiple classrooms and there is not enough time to converse with a teacher about the day's events.
