Daily Behavior Chart
This is a behavior chart that can be used daily. I use it for my students that have behavior issues. I go over it each day with the child and they help fill it out themselves. Feelings Chart
The student is able to identify how they are feeling: happy, sad, or mad. The chart prompts them to explain why they are feeling that way.
To avoid making copies the chart can be placed in a sheet protector. This way it can be written on and wiped off each week. Documentation can be kept by dating and taking a photograph of the chart.
