This form was created by me. I have used it in Kindergarten and 4th grade, so it can be used by all teachers. You can edit the comments, your name, school, parents' names, and everything but the clip-art. Save yourself an hour of time this year. It's everything you need to make your conferences a HUGE success!

Student Observation Log

This is an EXTREMELY simple form I keep in my binders for each class. There is a copy of this form for each student. I keep a clipboard of labels nearby during class, and when I note a behavior that I want to record, or if there is an incident in class, I record it on a label. Later when I have time, I stick the label on the observation form. Now my documentation is a piece of cake. If I need to talk to parents or administrators about a student's behavior in class, I just have to look at my log.
