Small Group Counseling Needs Assessment Form for Elementary Teachers
I use this resource to ask teachers to give information about which students in their classroom may benefit from group counseling sessions. I ask all the teachers in a particular grade level to fill out a form. When all teachers return the form I figure out which specific topic(s) is the biggest need for the grade level. Then I compile a list of students who would be a good fit for a group based on the returned forms.
