This word document is set up so that student assessment results can be collected and analyzed through out the school year. There are three sections, for Reading, ELA, and Math. Each one has three sub sections for student scores. This works great for MAP or other assessment programs.

I made these username and password cards so my students could independently use the technology lab. I plan to fill in student information, cut them out, and laminate them. They will then be made available to students when they are ready to use the classroom, media center, or technology lab computers. They could also be sent home so students could assess internet resources after school.

Function Tables (Input/Output Tables)

Set the Rule and then complete the Input/ Output Table. It can be used for addition, subtraction, multiplication, or division. This can be used with Input/ Output task cards or you can partly fill in the form your self to have the students figure out the rule and the missing numbers. Set a rule, and have the students complete the table with their choice of numbers. There are so many ways it can be adapted to meet the needs of your students. Enjoy!
